The Bordentown Regional School District processes all of our physical paperwork online, please adhere to the dates below. All information must be submitted through our website. Please scroll to the bottom of the page for Physical Due Date Timelines. Below are the instructions needed to begin the process:

Directions:

1. Go Click on the following LINK and complete the registration form.  This must be completed for each sport your child participates in.
2.  Please download your Pre-Participation Physical Form found in the “Athletic Forms” section and take this form to the physical appointment for the parent and physician to compete. Once complete, bring in the original physical forms into the nurse.
3. The third requirement to participate is turning in your “Health History Update” also found in the “Athletic Forms” section.
4. If your child requires athma clearance, epipen paperwork, or anyother medical clearences please turn them in prior to the start of any season.
5. Failure to turn in all required paperwork will prohibit your child from trying out.  To have your child cleared before the start of their season, please have all materials turned into the health office by the deadline listed below.

Physical Due Dates & Opening Days 2026-2027

Intramural Sports

Please remember - completed physical forms are needed for intramurals as well, forward completed forms to Mrs. Mathews. Also, there is no cut-off date for medical forms/permission slips to be turned in for intramurals. Please turn them in as soon as possible - your son/daughter will be cleared approximately two to three days after the form is submitted.

Please be advised, if you are seeking middle school athletic schedule information for a BRMS athletic team, and/or"GAME CHANGE AND CANCELLATION INFORMATION", please reference the link below.  Once on the site please go to the top of the page and click on the icon "Notify Me or Follow".  When changes are made, you will be notified in "real time" so that you are aware of any changes to your child(s)' schedule.
