**ON-LINE PHYSICAL REGISTRATION FOR ATHLETES**

The Bordentown Regional School District is excited to announce that we will begin processing all of our physical paperwork on-line.  All information submitted will be done through our website.  We understand that like any change, there will be positives and negatives.  It is our goal to help you work through this process as efficiently as possible.  We welcome your feedback on the new process and truly believe that this new process will be what is best for our students, their families, as well as the staff at BRSD.  Below are the instructions needed to begin the process:

  2. Please download your Pre-Participation Physical Form and take this form to the physical appointment for the parent and physician to compete.  Once complete, bring in the original physical forms to the nurse.

 
****Physical Examinations - IMPORTANT UPDATE BELOW:****

NEW ATHLETIC FORMS NOW IN USE -  Starting July 1, 2024, old physical packets will NO LONGER be accepted.  Please DO NOT submit the old physical packet after July 1, 2024.  They will not be accepted and you will be required to have your physician complete the new ones. This will increase the time needed for final clearance.  The form can be found below and on Health Services page under the "Health Office Forms" tab.

  1. Click onto  the Athletic Team Registration Icon to begin the on-line physical registration process.

Please forward any questions that you may have during the summer hours via email to the nurses or the athletic director as the nurses are not in school during the summer.

Every student needs a medical on file to participate in interscholastic athletics!

Intramural Sports

Please remember - completed physical forms are needed for intramurals as well, forward completed forms to Mrs. Mathews. Also, there is no cut-off date for medical forms/permission slips to be turned in for intramurals. Please turn them in as soon as possible - your son/daughter will be cleared approximately two to three days after the form is submitted.

Please be advised, if you are seeking middle school athletic schedule information for a BRMS athletic team, and/or"GAME CHANGE AND CANCELLATION INFORMATION", please reference the link below.  Once on the site please go to the top of the page and click on the icon "Notify Me".  When changes are made, you will be notified in "real time" so that you are aware of any changes to your child(s)' schedule.
