6th Grade Information

INCOMING 6TH GRADE STUDENTS FOR UPCOMING SCHOOL YEAR

MUST HAVE THE STATE MANDATED DTP BOOSTERS AND MENINGITIS IMMUNIZATIONS PRIOR TO THE START OF THE 6TH GRADE SCHOOL YEAR.

***Due to the circumstances surrounding COVID-19, please make every attempt to schedule physical and immunization appointments! As of now there will not be extensions given for the new school year. ***

In accordance with the New Jersey Administrative Code 8:57-4 and the Department of Health and Senior Services, the following changes have been made effective September 1, 2008; students born after January 1, 1997 and entering 6th grade are REQUIRED to receive on booster of the Tdap (tetanus, diphtheria & pertussis) vaccine and one dose of the Menactra (meningococcal) vaccine.

Please contact me with any further questions in regards to the required immunizations.  During the summer hours the Health Office is closed, please direct any questions via email, as I respond to my emails throughout the summer.

Height, weight, blood pressure and vision screenings are performed for every 6th grade student by the school nurse as indicated by N.J.A.C. 6A16-2.2

It is recommended that as you have your child's yearly physical exam at the physician's office, download a sports physical packet, forms are located under the "Health Office Forms" tab to the right of this page. Have your physician complete  pages 3-5 of the sports physical packet at the time of the visit. This could alleviate any unnecessary trips back to the physician's office for paperwork if your child chooses to participate in an intramural or interscholastic sport for the middle school each year.
