8th Grade Information

Height, weight, blood pressure and vision screenings are performed for every 8th grade student by the school nurse as indicated by N.J.A.C.6A:16-2.2 .

It is recommended that as you have your child's yearly physical exam at the physician's office, download a sports physical packet, forms are located under the "Health Office Forms" tab to the right of this page. Have your physician complete  pages 3-5 of the sports physical packet at the time of the visit. This could alleviate any unnecessary trips back to the physician's office for paperwork if your child chooses to participate in an intramural or interscholastic sport for the middle school each year.
